Assistant Clinical Program Manager At Bayshore HealthCare

Location: Mississauga, Ontario

Job Description

JOB SUMMARY
This position will focus on the management of case managers (nurse staff), education, day to day activities, and implementation of new program entities. The Assistant Clinical Program Manager reports directly to the Clinical Program Manager.

DUTIES AND RESPONSIBILITIES

  • Responsibility is to be the Interim Clinical Program Manager for the first year.
  • Oversees the day to day assignment and operation of the nurse case management staff
  • Ensures all program case management performance expectations are monitored and sustained by completing monthly telephone and documentation audits for case managers
  • Implements and evaluates new program initiatives that impact the case managers
  • (i.e. new drugs and program changes)
  • Completes investigations for case manager QMRs and escalates issues and complaints to the Sr. Operational Manager as required.
  • Provides input into program case manager gaps requiring development
  • i.e. CRM
  • Maintains case manager staffing model based on workload
  • Manages escalations of referral workload with the Assistant Intake Program Manger
  • Participates in program checkpoint meetings and DERC
  • Provides ongoing orientation to new case manager staff
  • Supports case manager staff in meeting performance expectations and completes annual reviews
  • Steps into role of case manager as required
  • Supports achieving creating a positive work environment working within a team based structure.
  • Supports administrative processes including: monitoring of data integrity;
  • Ensures proper certification/recertification procedures are in place including PV requirements for program

Qualifications:
QUALIFICATIONS & EXPERIENCE

  • Three to five years of management experience
  • Demonstrated ability to work with people across disciplines/departments
  • Excellent computer skills including Microsoft suite of products
  • (minimum intermediate level Excel, Word, Power Point)
  • Strong organizational skills
  • Exceptional written and oral communication skills
  • Registered Nurse (RN) License – BScN or currently working towards degree –required in good standing with the College of Nurses
